Financing Information and Payment Options

As a condition of treatment by this office, financial arrangements must be made in advance. We depend upon timely reimbursement from patients for the costs incurred in their care. Therefore, the financial responsibility on the part of each patient must be determined before treatment. We will discuss payment options with you before rendering treatment.

All patients financial requirements are treated on an individual basis. Please feel free to discuss any options that are not covered below. Dr. Bracco is fully understanding of today’s economic climate and where possible will always try to come to a mutual beneficial arrangement. A general outline of payment options available to you are as follows:

Payment Policies

We accept payment by cash, check and credit cards (Visa, MasterCard and American Express).

Interest-free financing through Care Credit

For treatment plans over $500 you can apply for an interest free loan with fixed payments for a period of 3, 6 and 12 months. Longer terms are also available. We can give you an instant approval with a simple credit application. For more information visit carecredit.com we also accept most PPO insurances; please call our office to verify your eligibility.

There is a courtesy fee of 7% for cash/check payments and 3% for payments made by credit card. Payment is due full at time of actual treatment. If you have dental insurance, please see our policy below.

Dental Insurance

Dental insurance is designed to help you offset some of the cost of dental treatment. It is very different from your medical insurance. Generally, dental insurance is subject to a deductible and pays for only part of your treatment up to a modest annual maximum limit depending on the specific provisions of your plan. Our policies regarding dental insurance are as follows:

1. We will gladly file insurance claims for you to help you maximize your benefits. However, you are ultimately responsible for any amount not covered by your insurance.

2. On your behalf, we will contact your insurance company to determine your level and type of benefits prior to any scheduled treatment.

3. In order to make your treatment as predictable and efficient as possible, we will generally submit a pre-treatment authorization to your insurance company. They will send back a statement that explains exactly the amount they will pay for a given procedure. The difference between our fee and what your insurance company pays is your out–of–pocket expense.

4. We will generally accept assignment of benefits (payment) directly from your insurance company but we reserve the right to refuse assignment on certain plans. In this case, full payment is due by you at the time of service and your insurance company will reimburse you directly.

5. If you have dental insurance and we agree to accept assignment of benefits from your insurance company, your portion of payment is due in full at the time of service.

6. Remember, your insurance policy is a contract between your employer and your employer’s insurance company—we are not party to that agreement. Therefore, our office cannot accept responsibility for negotiating a settlement with your insurance company on a disputed claim.
